Who needs D4 form?

DVLA require D4 for
  • All first applicants for a provisional group 2 bus driving licence.
  • Existing Lorry drivers at the age of 45 must take the bus driver’s medical examination and then every five years until the age of 65.
  • After 65, the bus driver’s medical assessment must be done every year.
A Group 2 licence is required for HGV/LGV, Bus, Minibus and PVC drivers (D1 category), Coach, Caravanes and Ambulance drivers (C1 category)
